How to register as a patient

Please come into the Surgery to register (Get directions), when we will check your address as we will only accept new patients living within our Practice area . (If you wish to check you are living in our Practice area, please go to the Practice area link on this website or telephone to enquire: 01206 864395). Please bring your NHS medical card with you, if you can find it, otherwise you will be asked to complete a registration form by the Receptionist- if you wish you can download this form herepdf file symbol and fill it out in advance. There will also be other forms to complete as appropriate to each patient, eg New Patient Questionnaire, child immunisation consent, child health surveillance, etc.

We ask that all new patients to the Practice over 44 years of age and new patients over 5 years of age and taking regular medication, including contraception and regular injections attend for a New Patient Health Check with our Health Care Assistant. This involves a brief check of height, weight, blood pressure, urine testing. Your medical history will also be taken so that we have some of your details available to us, as it can take several weeks to get your complete records through from your previous Practice.

The following new patient groups will not be required to undergo a New Patient Health Check:
- Newborn to 5 year olds
- 5 to 44 year olds not taking regular medication (but a urine sample will be required for testing)

It is best that you register and book your New Patient Health Check (if required) as soon as possible when you need to change Doctors, as ideally we prefer this to have been done before it is necessary for you to see a Doctor or Nurse or need to make an appointment.
