These letters will incur a fee from £ 15.00. The letter will usually take up to 4 working days to be ready for collection, but if you need it sooner please inform the Doctor at the time of request. When you collect the letter from Reception, you will be asked for the fee at this time. We can only accept cash or a cheque for payment (no credit cards). Other Services attracting Private Fees Adoption Medical Examination Driving Licence Photocard Applications Employment/pre-employment Medical Examination Fitness to travel certificate HGV, PSV & Fitness to drive Medical Examination Holiday Cancellation Forms Passport signing (only carried out by Dr Sampson) Private Insurance Medical Claim Form, ie BUPA, PPP, etc Private Sickness Certificate Sports Medical Examination Travel Vaccinations/Prescriptions/Certificates Vaccination Status Letter Please ask at Reception for the current fees for the above. Please note as medical examinations require long appointments you must give 24 hours notice to cancel the appointment if you are unable to attend. Failure to attend an appointment for a private medical examination, without adequate notice, will incur a charge of 50% of the fee for the medical examination, for which you will be invoiced. Drop-in Clinic For Breast Prosthesis Ladies requiring new breast prostheses or partial breast form if they have a discrepancy in breast size following breast surgery, are welcome to attend a Drop-in Clinic held every Wednesday, between 1.30 and 4.00 pm at the Outpatient Department, Essex County Hospital.
