Patients who are due to travel abroad and who might require vaccinations or travel advice are encouraged to make an appointment by contacting the Medical Centre. Appointments should be made 8 weeks before departure as occasionally 2 vaccinations are required to be given 4 weeks apart to provide adequate protection. Most vaccinations take at least 14 days to take effect so if given too near the departure date you will not be protected on your holiday. Appointments are limited and we may not be able to help if last minute holiday bookings are made.
We request that patients fill in a simple form, outlining their travel plans and indicating which vaccinations they have already received (if known), prior to attending the clinic. This will enable our nurses to plan the appointment in the most efficient way.
You can obtain this form in a number of ways :
Collect the forms (one for each person) from reception at Parsons Heath Medical Practice.
Download the form (one for each person), complete it and either drop it in to the surgery or post it
Travel Questionnaire![]()
Complete the form here -again remember a form has to be completed for each person travelling (this is not encrypted so if you have security concerns bring or post the form to the surgery in the usual manner).
