Please only use this form for compliments or comments about the Practice and suggestions as to how we can improve our service to you.   The following CANNOT be dealt with via this form: Complaints, Medical matters, Appointment queries/cancellation or Prescription requests/problems. We aim to provide a good quality friendly care for our patients and are pleased to hear what you think of the service.  If you like what we do please let us know as it cheers us up.

If you are not happy with the service please speak to the Practice Manager.  We will do our best to respond to complaints quickly and to sort out the problem out.  If you wish to make a complaint please take a Patients Complaint Information Leaflet, which is available  in both the waiting areas.  This will tell you how to proceed with your complaint, which should be addressed to Mrs Jackie Hawkins, Practice Manager.
